This episode tackles a question that matters in daily practice but is rarely answered cleanly: what actually happens with electrotherapy over the long term? Not after a week – but after a full year. To explore it, we look at two brand-new scientific papers from 2026. One goes deep into the single cell; the other follows real patients across twelve months. Together they paint a surprisingly clear picture: short-term, a lot looks the same – the decisive difference only emerges over time. We break down the difference between TENS and microcurrent (MENS), why one dampens the pain signal via the nerve while the other works a level deeper at the cell, and what that could mean for long-term tissue regeneration. In this episode: TENS and microcurrent – how they fundamentally differ Study 1: how microcurrent dials down the "master switch" of inflammation at the cellular level Study 2: a 12-month head-to-head of microcurrent and TENS at the shoulder The "clinico-radiological paradox" – when symptoms and tissue structure move in opposite directions Why the real benefit often only shows up months later What this means for the interplay of exercise therapy and microcurrent Sources for this episode: Wang, Z.-L., Li, H.-B., & Li, Y.-T. In this particularly in-depth episode, we translate ‘big’ concepts such as entropy, enthalpy and free enthalpy (ΔG) into clinical practice: what do they mean for transport processes, ion flows, microcirculation and ATP supply in tissue? You’ll learn how the Luxxamed algorithm uses a closed-loop system to first measure (electrical tissue response/impedance), then evaluate (physiological target range) and finally dynamically adjust microcurrent and LED output – always taking acute versus chronic patterns into account. What you’ll learn in this episode ️ Cybernetics in the body: Why ‘measure evaluate regulate’ is the clinically sound approach. ️ Thermodynamics made simple: entropy/enthalpy/ΔG as practical conceptual tools for acute ‘hot’ versus chronic ‘rigid’ tissues. ️ Cellular basis: ion gradients, membrane potential, ATP – and how microcurrent acts in a regulatory rather than a stimulating manner. ️ Electro-conformational coupling: Why very small currents (µA) can influence protein functions. ️ Photobiomodulation: Light as a co-regulator of the respiratory chain (with purpose and in moderation). ️ Algorithm in practice: What is measured, how the response curves are interpreted, and how this leads to an adaptive dose. ️ Clinical application: Approaches to acute inflammation, chronic stiffness, early post-operative rehabilitation and sports recovery. ️ Guidelines & safety: Sub-sensory dose, electrode bracketing, treatment frequency, hydration, co-therapies, operator responsibilities.

Why ‘Copy + Paste’ in microcurrent therapy quickly turns into ‘Copy & Waste’ – and how to use electrode set-ups individually rather than by the book. This episode tackles one of the most common uncertainties in clinical practice: what to do when the standard electrode configuration simply isn’t working? We’ll look at why a configuration is always just an idea, not a set formula – and how to choose local and global strategies in a conscious and well-reasoned way. Here’s what you can expect: ️ Why “copy and paste” in electrode configuration quickly becomes “copy and waste” ️ The knee example: why a correctly positioned electrode placement can still be ineffective if the cause lies in the biomechanical chain (hip–knee–foot) ️ The crucial difference between microcurrent and conduction anaesthesia – and what this means for your treatment decision ️ Three different schools of electrode placement: the symptom-oriented DaWoS method, the systemic approach (Matthias Rother) and the regulatory concept (Burkhard Hock) with its three basic cellular conditions ️ The Tensegrity model: Why the body is not a collection of individual parts, but a continuously interconnected system of tension ️ Historical digression: Robert Becker, ‘The Body Electric’ and the myth surrounding red/black polarity ️ Practical knowledge: galvanically isolated channels, light channel allocation, conductive gloves and socks as an alternative to electrodes ️ Four specific recommendations for your day-to-day practice Scientifically underpinned by recent studies on central sensitisation, myofascial chains, cellular inflammatory mechanisms and electrode placement (full list of sources in the blog article).
